It is common to have hard scarring after anal fistula surgery, especially within the first three months post-operation. These scars usually soften gradually over time and typically do not require special treatment. If the scar remains hard after three months, you may try sitting in warm water (sitz bath) or applying a warm towel compress two to three times daily for about 20 minutes each time to help soften the scar tissue. Under a doctor's guidance, topical medications that promote tissue regeneration and reduce inflammation—such as hemorrhoid ointment or Jiuhua ointment—can be applied to the scar area to aid in softening the scar and promoting wound healing. It is important to avoid constipation or diarrhea, maintain healthy bowel habits, eat a light and easily digestible diet, and avoid spicy or irritating foods to minimize irritation to the intestines and surgical site.
