The main treatments for endometriosis include drug therapy and surgical treatment.
1. Drug Therapy:
Progestins: Such as medroxyprogesterone acetate, which induce a state of pseudopregnancy to halt menstruation and cause the atrophy of ectopic endometrial tissue, thereby achieving therapeutic goals.
Prostaglandin synthesis inhibitors: Such as naproxen sodium, suitable for patients with significant dysmenorrhea who are not candidates for surgery or hormonal therapy.
Androgenic agents: Such as methyltestosterone, which have particularly noticeable effects on dysmenorrhea caused by endometriosis, but dosage must be strictly controlled.
2. Surgical Treatment:
Conservative surgery: For patients who wish to preserve fertility, such as uterine suspension or excision of ovarian endometriomas.
Semi-conservative surgery: For patients without fertility requirements, under 45 years of age, or with extensive lesions; involves removal of the uterus and endometriotic lesions.
Radical surgery: For patients with very severe disease or those approaching menopause; involves complete removal of the uterus and adnexa, without preservation of ovarian function.
