The main causes of pain in the bone above the female gluteal cleft include the following aspects:

1. Traumatic factors: such as falling from a height, violent impact to the coccygeal area, or sitting down forcefully, which may cause coccygeal injury or fracture, resulting in pain. This is a relatively direct and common cause, and severe injuries may even require surgical treatment.

2. Prolonged sitting and strain: sitting on a hard stool for extended periods can cause repeated friction between the coccygeal tip and hard objects, possibly leading to bursitis or local ischemia, edema, inflammation, and subsequent pain. Additionally, chronic strain and lumbar spine conditions such as the third lumbar transverse process syndrome may also affect the coccygeal region, causing pain.

3. Gynecological diseases: such as pelvic inflammatory disease and adnexitis, where the inflammation may radiate to the coccygeal area, causing pain.

4. Degenerative changes: with aging, degenerative changes may occur in the coccyx and surrounding tissues, such as bone spurs, which may also cause pain.
