Testicular hydrocele surgery is performed as follows:

1. Anesthesia: Prior to the start of the surgery, the patient must receive either general or local anesthesia.

2. Incision placement: An appropriately sized incision is made on the scrotal skin, usually at the site where the fluid accumulation is most apparent, to facilitate the procedure and minimize tissue damage.

3. Exposure of the tunica vaginalis cavity: The tunica vaginalis cavity is accessed through the incision, and the tunica vaginalis is separated and exposed.

4. Examination and drainage: The physician examines the testicle and tunica vaginalis to determine the extent and location of the fluid accumulation. A syringe is then used to aspirate the fluid and completely drain the accumulated liquid from the tunica vaginalis cavity.

5. Management of the tunica vaginalis: Depending on the specific condition, the surgical approach is selected. For patients with a thin tunica vaginalis, plication suturing is performed. For patients with significant fluid accumulation, a tunica vaginalis inversion procedure may be used, in which the inner layer of the tunica vaginalis is turned outward and sutured in place to reduce the likelihood of recurrence.

6. Closure of the incision: Upon completion of the surgery, the incision is closed, dressed with sterile gauze, and wrapped with a bandage.
