Elevated cortisol levels in children can be caused by various factors, mainly including the following:

1. Adrenal gland issues: Adrenal cortical tumors are also common causes of elevated cortisol levels in children. These tumors may secrete excessive cortisol, leading to increased cortisol levels in the blood. Adrenal gland lesions can cause increased cortisol secretion.

2. Dysfunction of the pituitary and hypothalamus: Excessive secretion of ACTH (adrenocorticotropic hormone) by the pituitary gland, or hypothalamic dysfunction, may also lead to elevated cortisol levels in children. ACTH is the main hormone that stimulates the adrenal cortex to secrete cortisol, and abnormal secretion can directly affect cortisol levels.

3. Long-term medication effects: Long-term use of ACTH or cortisol medications may also lead to elevated cortisol levels in children, a condition clinically referred to as exogenous Cushing's syndrome.
