Pain under the right rib cage can be caused by various factors:

1. Diseases of the ribs and surrounding tissues: such as rib fractures, costochondritis, and intercostal neuralgia, which can directly affect the ribs and surrounding tissues, causing pain.

2. Liver and biliary diseases: such as cholecystitis, gallstones, hepatitis, and cirrhosis. Since the liver and gallbladder are located in the right subcostal region, these diseases often manifest as pain in this area.

3. Gallbladder diseases: such as cholecystitis and cholelithiasis, which are often accompanied by right upper quadrant abdominal pain that worsens after eating fatty foods and sometimes radiates to the right shoulder or back.

4. Kidney diseases: such as kidney stones and pyelonephritis. The pain is usually located in the flank area but may sometimes radiate downward to the subcostal region.

5. Pancreatic diseases: acute or chronic pancreatitis. Although it mainly presents as upper abdominal pain, the discomfort can sometimes radiate to the left or right subcostal area.
