654-2, also known as anisodamine, is an M-cholinergic receptor antagonist commonly used to relieve smooth muscle spasms, improve microcirculation, and more. The use of anisodamine in patients with acute pancreatitis should be undertaken with caution.
The treatment principle for acute pancreatitis is to reduce pancreatic secretion and inflammatory responses. Anisodamine may affect pancreatic secretory function by inhibiting cholinergic nerve activity, which theoretically could reduce the burden on the pancreas.
