What to do about severe edema in membranous nephropathy
Recently, I have been experiencing persistent pain in my kidney area, noticed blood in my urine, and developed swelling in my feet. My doctor diagnosed me with edema caused by membranous nephropathy. I would like to know how this condition should be managed?

Membranous nephropathy is a common primary glomerular disease, characterized pathologically by subepithelial immune complex deposition in the glomerular basement membrane, leading to basement membrane thickening. The following are some treatment recommendations:
1. Dietary adjustment: Reduce sodium intake and moderately consume foods rich in vitamins, such as tomatoes and carotenoids. Ensure adequate intake of high-quality proteins, such as milk and eggs.
2. Medication: Diuretics, such as spironolactone, help reduce fluid retention. Glucocorticoids, such as methylprednisolone, alleviate kidney inflammation and proteinuria, thereby improving edema.
3. Blood pressure control: Combine lifestyle modifications with medication, such as antihypertensive drugs nifedipine and captopril, to reduce the burden on the kidneys.
4. Appropriate physical activity: Engage in mild activities, such as walking, to promote blood circulation, but avoid strenuous exercise.
5. Active management of complications: Treat complications such as hypertension and hyperlipidemia to comprehensively control the disease progression and reduce the burden on the kidneys.
6. Regular follow-up: Monitor changes in kidney function and adjust the treatment plan timely.
