Recently, I noticed a ring of small yellowish spines around my anus. Initially, I thought it was a common skin issue and did not pay much attention. However, when washing the area with soap, I experienced sharp, needle-like pain, causing significant discomfort. Now, I am very concerned whether this could be a serious condition and urgently want to know what it is and how it can be treated.

Yellowish small spines appearing around the anus accompanied by needle-like pain may indicate a variety of perianal diseases. Common causes include hemorrhoids, anal fissures, perianal eczema, or genital warts. Firstly, this could be perianal eczema secondary to hemorrhoids. Hemorrhoids are a common anorectal condition, and perianal eczema is one of its common complications, manifesting as redness, papules, vesicles, and other skin lesions around the anus, which may be accompanied by itching and pain. Secondly, folliculitis is also a possible cause, especially when there is infection by Staphylococcus aureus, which can cause inflammatory reactions in the local hair follicles, resulting in red papules and pain.
For this symptom, patients should immediately stop using soap to wash the anal area, because some irritating ingredients in soaps may exacerbate skin discomfort. It is recommended to gently cleanse the area with warm water instead. Patients should also promptly consult a dermatologist or proctologist to obtain an accurate diagnosis and appropriate treatment. During treatment, patients should keep the anal area clean and dry, avoid scratching and friction to prevent worsening of the condition. Additionally, attention should be paid to maintaining a healthy diet, consuming more fiber-rich foods, ensuring smooth bowel movements, and avoiding constipation to reduce pressure on the anal area.
