In general, whether a CT scan without contrast can detect pancreatic cancer mainly depends on the stage of the disease. If obvious symptoms are already present, it is usually detectable. However, in cases of early-stage pancreatic cancer where symptoms are not apparent, detection may be difficult.

A CT scan without contrast provides cross-sectional images of internal body structures and can identify larger or more prominently located pancreatic masses, offering a certain degree of sensitivity. When pancreatic cancer is advanced, with relatively large tumor lesions, and the patient has developed clear symptoms such as abdominal distension or pain, a CT scan without contrast can usually detect the condition.

However, due to the lack of noticeable symptoms in early-stage pancreatic cancer and the possibility that distinct tumor lesions have not yet formed in the pancreas, false-negative results may occur with CT scans without contrast.
