When the gestational sac develops rapidly, high levels of estrogen and progesterone in the body stimulate the breasts, leading to mammary duct proliferation and causing breast pain and swelling starting around week 4 of pregnancy. If the gestational sac develops more slowly, breast swelling may start after week 9, or may not occur at all during pregnancy.
