Introduction
Cough with green purulent sputum suggests a decline in the body’s immune resistance, leading to infection by *Pseudomonas aeruginosa* and subsequent inflammatory response of the respiratory mucosa. This results in mucosal hyperemia, edema, and copious exudation of purulent, greenish secretions, which are expelled from the body via coughing and expectoration. Such symptoms are commonly observed in conditions including acute bronchitis, acute exacerbation of chronic bronchitis, pneumonia, and bronchiectasis.

Coughing up phlegm is a normal physiological process. For many people, expelling phlegm requires no special concern; however, upon closer observation, you may notice variations in phlegm color. So, what does it mean when coughing produces thick green phlegm? Below is an explanation.

What Causes Cough with Thick Green Phlegm?

Cough accompanied by thick green purulent sputum typically indicates decreased host immunity and infection with Pseudomonas aeruginosa, leading to inflammatory responses in the respiratory mucosa. This results in mucosal congestion, edema, and excessive exudation of purulent green secretions, which are subsequently expelled via coughing. The sputum contains large numbers of viable and dead Pseudomonas aeruginosa bacteria, desquamated tissue fragments, and inflammatory metabolic byproducts. Such findings are commonly observed in conditions including acute bronchitis, acute exacerbation of chronic bronchitis, pneumonia, and bronchiectasis. Appropriate antimicrobial therapy should be initiated using antibiotics to which the pathogen is sensitive—such as cefoperazone-sulbactam, ceftazidime, ciprofloxacin, tobramycin, or meropenem. However, patients experiencing severe cough should seek prompt medical evaluation and treatment at a hospital. During treatment, patients should also pay attention to dietary habits—avoiding spicy and irritating foods—and abstain from smoking and alcohol consumption to prevent adverse reactions.

Knowledge Extension: How to Relieve Cough

1. For severe or frequent coughing, patients should take antitussive medications correctly under physician guidance. Drinking an appropriate amount of warm water daily helps moisten the respiratory tract and alleviate coughing.

2. Ensure the patient’s environment has clean, fresh air with suitable temperature and humidity. A humidifier may be used if necessary to relieve cough symptoms. Additionally, avoid consuming spicy, cold, or irritating foods, and minimize exposure to allergens such as tobacco smoke, pollen, fragrances, and other pungent odors.
