What causes abdominal cramping and pain at six months of pregnancy?
By six months, pregnancy has entered the second trimester. At this stage, the baby has grown significantly within the mother’s uterus and the pregnancy is generally considered stable. Compared to early pregnancy, complications have markedly decreased; however, expectant mothers at six months gestation must still remain vigilant and avoid strenuous physical activity. So, what causes cramp-like abdominal pain at 26 weeks?

What Causes Cramp-Like Abdominal Pain at Six Months’ Gestation?
First, abdominal pain at six months may result from uterine enlargement and consequent stretching of the round ligaments of the uterus. This type of pain typically occurs in the lower abdomen—often on one or both sides—and presents as a dull, aching, or referred discomfort. It is considered physiological (normal) and usually requires no specific intervention; rest in bed typically provides relief.
Second, external physical stimuli may trigger abdominal pain. During pregnancy, elevated sex hormone levels increase blood flow to the reproductive organs, rendering them more susceptible to injury and bleeding. As the uterus and abdominal cavity are already in a state of physiological hyperemia, external trauma—such as blunt impact—or overly vigorous sexual intercourse may provoke abdominal discomfort. In such cases, rest and quiet recuperation are advised.
Third, spontaneous abortion (miscarriage). Although the risk of miscarriage declines substantially by the second trimester, it remains possible—particularly if fetal chromosomal or genetic abnormalities are present.
Fourth, gastrointestinal discomfort. Pregnant women often experience relative physiological weakness, necessitating heightened attention to overall health. Consuming spicy, expired, or excessively cold foods—or exposing the abdomen to cold temperatures—may irritate the gastrointestinal tract, leading to spasms and associated abdominal pain.
