Most elderly individuals experience hand tremors, and when severe, these tremors can significantly impair their quality of life. What, then, are the underlying causes of hand tremors in older adults?
Causes of Hand Tremors in Older Adults
Hand tremors in the elderly generally fall into several categories. First, psychological factors—such as mental stress, anxiety, depression, or poor sleep—can trigger tremor symptoms. Psychological counseling and appropriate symptomatic treatment often lead to noticeable improvement. Second, essential tremor—a typically autosomal dominant inherited condition—may also present with head tremor. If there is a family history, medications such as propranolol may be effective. Third, Parkinson’s disease or parkinsonian syndromes are characterized not only by resting tremor of the hands but also by muscle rigidity, bradykinesia (slowness of movement), and even autonomic dysfunction.

Diagnosis requires brain MRI imaging, and dopaminergic medications are commonly used for treatment. In cases where parkinsonism arises secondary to cerebrovascular disease (vascular parkinsonism), encephalitis, or carbon monoxide poisoning, management must target the underlying etiology. Additionally, certain medications—including flunarizine, antiemetics such as metoclopramide, and psychotropic agents like olanzapine and risperidone—can induce drug-related tremors; dose reduction or discontinuation usually alleviates the symptom.

Non-pharmacological interventions—including physical therapy and targeted exercises—can also help manage hand tremors in older adults. For instance, repetitive hand movements during tremor episodes may improve motor coordination and dexterity. Pharmacological treatment, however, must be guided by a physician’s definitive diagnosis; self-medication is strongly discouraged, especially given the high prevalence of polypharmacy among older adults.
