Anal fistulas typically cause swelling and pain around the anus, with pain often intensifying during bowel movements. They may also lead to purulent discharge from the surrounding skin and cause pruritus (itching) of the perianal skin. In severe cases, recurrent local inflammation and infection may occur, potentially resulting in systemic symptoms such as constipation, anemia, and weight loss.
