The most common initial sites of hematogenous metastasis are the chest wall and lungs—with pulmonary metastasis being the most frequent. Patients may experience chest pain or unexplained coughing. In some cases, distant metastasis to the liver may already have occurred but remain clinically undetectable without specialized evaluation. Diagnosis therefore often requires clinical assessment by a physician. Early-stage patients may exhibit elevated serum tumor markers. During outpatient referral, patients should fast prior to undergoing relevant tests—including ultrasound and contralateral mammography—to assess for possible metastasis to the opposite breast.

Firstly, breast cancer commonly spreads via the lymphatic system. During follow-up visits, it is essential to determine whether any lymph nodes on the affected side—either previously non-enlarged or missed during initial surgery—have since become involved.
