What should be done for cirrhosis with splenomegaly and ascites?
Once this condition is detected in patients with cirrhosis, it is often already quite advanced and poses significant health risks; therefore, it warrants serious attention and prompt initiation of appropriate treatment. However, some individuals may be unclear about how to manage cirrhosis after its diagnosis. First and foremost, selecting the correct therapeutic approach is essential—so what should be done when cirrhosis is accompanied by splenomegaly and ascites?
Management of Cirrhosis with Splenomegaly and Ascites
Cirrhosis-associated splenomegaly and ascites can be managed through conservative therapy, antifibrotic treatment, and other modalities. Conservative management is the first-line approach for both cirrhosis and splenomegaly. Following initial conservative treatment, subsequent therapeutic strategies should be tailored according to the underlying etiology. For instance, if cirrhosis results from hepatitis B virus (HBV) infection, nucleos(t)ide analogues are recommended as first-line antiviral agents to achieve effective viral suppression. Secondly, since hepatic fibrosis typically precedes cirrhosis—and much of this fibrosis remains reversible—antifibrotic therapy is a viable option. Additionally, hepatoprotective therapy may be considered; however, when selecting such therapy, liver function—particularly serum albumin levels—must be taken into account.

If splenomegaly is pronounced, surgical splenectomy may be considered to achieve therapeutic benefit. Following splenectomy, blood sequestered within the enlarged spleen no longer accumulates there, thereby effectively reducing portal venous pressure. Concurrently, hypersplenism gradually resolves, which helps alleviate anemia. When managing cirrhosis with splenomegaly, dietary supplementation with fresh vegetables or fruits rich in vitamin C is recommended as an initial supportive measure.

In daily life, patients should adhere to a disciplined diet—avoiding spicy, irritating, or overly rich foods, refraining from binge eating, and ensuring adequate rest by avoiding late-night activities. We hope this information proves helpful to you.
