Essentially, “yin deficiency with internal heat” and “yin deficiency with blazing fire” represent the same underlying pathological mechanism—there is no fundamental distinction between them. Both may manifest with symptoms such as heat sensation in the palms, soles, and chest (“five-center heat”), night sweats, tinnitus or hearing loss, a red tongue with scant or absent coating, and dry mouth and throat. However, “blazing fire” reflects a more severe stage of yin deficiency. Since health depends on the dynamic balance between yin and yang, yin (often metaphorically represented by water) and yang (by fire) must remain proportionally harmonious. When yin (water) becomes depleted, yang (fire) appears relatively excessive, resulting in internal heat. With further yin depletion, this relative excess escalates into “blazing fire,” presenting more intense signs such as bone-steaming fever, tidal fever, a bright-red, glossy tongue with little or no coating.
