Polycystic usually refers to polycystic ovary syndrome (PCOS). The detection of more than 20 follicles on ultrasound does not necessarily confirm PCOS or indicate severe PCOS. Simply observing over 20 follicles in a woman's ovaries via ultrasound requires further evaluation based on menstrual history for accurate diagnosis.
In PCOS, finding more than 20 follicles bilaterally during an ultrasound does not imply more severe disease. The number of 2–9 mm follicles observed via gynecological ultrasound does not correlate with the severity of the condition. As long as there are 12 or more such small follicles in one or both ovaries, the diagnosis may be considered; however, having more or fewer follicles does not reflect disease severity.
There is no standardized grading system for the severity of PCOS itself. Clinical presentations vary significantly among individuals, with considerable differences in symptoms. Some patients have metabolic issues, others may have hyperandrogenism without metabolic abnormalities, and some might also present with hyperprolactinemia. Therefore, clinical manifestations of PCOS differ widely among affected women.
