Introduction
Bilateral ovarian enlargement refers to ovaries on both sides that are larger in volume than normal. This condition may be a normal physiological response, or it could result from pathological factors such as endometritis, ovarian cysts, or polycystic ovary syndrome (PCOS). Bilateral ovarian enlargement may be related to the female menstrual cycle, as ovarian size can change at different stages of the cycle.

Bilateral ovarian enlargement refers to both ovaries being larger than normal in size. This condition may be due to normal physiological changes, or it could result from pathological factors such as endometritis, ovarian cysts, or polycystic ovary syndrome (PCOS). It is recommended to seek medical attention at a hospital as early as possible and undergo appropriate examinations followed by active treatment.

1. Physiological Causes

Bilateral ovarian enlargement may be related to the female menstrual cycle. Ovarian size naturally varies during different phases of the menstrual cycle, so ultrasound examinations performed at different times may show ovaries of varying sizes.

2. Pathological Factors

1) Endometritis

If a patient does not maintain proper hygiene before or after sexual intercourse, endometritis may develop. Increased secretions can wrap around the ovaries, leading to ovarian enlargement. Some patients may also experience abdominal pain. Anti-inflammatory medications can be used under a doctor's guidance for treatment.

2) Ovarian Cysts

When cystic masses develop in the ovaries, they typically present clinically as mobile, painless intra-abdominal lumps. Ultrasound imaging will show enlarged ovaries on both sides. Treatment may involve oral medications such as drospirenone-ethinyl estradiol tablets, desogestrel-ethinyl estradiol tablets, or ethinyl estradiol-cyproterone acetate tablets, as directed by a physician.

3) Polycystic Ovary Syndrome (PCOS)

Abnormalities in endocrine function and metabolism can lead to PCOS, which clinically presents with symptoms such as amenorrhea, hirsutism, obesity, and infertility. Over time, bilateral ovarian enlargement may develop. Under medical supervision, medications such as dexamethasone, cyproterone, spironolactone, and cyproterone acetate may be prescribed.

In addition to the above causes, conditions such as oophoritis (ovarian inflammation) and ovarian tumors may also lead to bilateral ovarian enlargement. It is advisable to promptly visit a hospital for evaluation and receive medication under a doctor’s guidance.
