What does it mean if the due date has passed but labor hasn't started yet?

Introduction
Fetal body measurements such as head circumference, abdominal circumference, and femur length, as well as fetal weight, are influenced by gestational age; therefore, different fetuses may be born at different times. The due date is merely the most likely time calculated based on typical cases, so it is still normal for delivery to occur after the due date. In addition, fetal asphyxia could also be a possible cause.

Post-term pregnancy without delivery may result from physiological factors influenced by gestational age, or could be caused by pathological conditions such as placental dysfunction or preeclampsia. The specific analysis is as follows:

I. Physiological Factors

Fetal body measurements such as head circumference, abdominal circumference, and femur length—and consequently fetal weight—are influenced by gestational age. Therefore, the timing of birth varies among different fetuses. The due date is merely an estimate based on typical cases, so it is still possible to have a normal delivery after the due date has passed.

II. Pathological Factors

1. Placental Dysfunction

The fetus depends on oxygen and nutrients supplied by the mother, which are delivered via the placenta. If placental function becomes abnormal, it can lead to placental insufficiency, reducing the supply of oxygen and nutrients to the fetus, resulting in delayed fetal development and post-term pregnancy. In such cases, pregnant women should go to the hospital for evaluation to confirm whether placental dysfunction exists and receive timely treatment to prevent more serious pregnancy complications.

2. Preeclampsia

Preeclampsia is one of the common complications during pregnancy. Due to factors such as hypertension, renal insufficiency, or hypothyroidism, toxins may accumulate in the pregnant woman’s body, leading to elevated blood pressure and symptoms including headache, blurred vision, and loss of appetite. If a pregnant woman develops preeclampsia, the fetus may also be affected, with impaired growth and development, potentially leading to post-term delivery. In such cases, the hospital will decide whether labor should be induced based on the condition of both mother and fetus.

In addition to the above, fetal distress may also be a cause. It is recommended to seek medical attention promptly and receive targeted management under a doctor's diagnosis.
