What is the management approach for cervical canal pregnancy?
In general, cervical canal pregnancy refers to the implantation of a gestational sac within the cervical canal and is a type of ectopic pregnancy. Management options include medical treatment, interventional therapy, and surgical treatment, as detailed below:
1. Medical Treatment
If there is minimal or no vaginal bleeding associated with cervical pregnancy, medication may be prescribed under a doctor's guidance. Examples include methotrexate tablets, mifepristone tablets, and Chanfukang granules. Appropriate use of these medications can suppress trophoblastic cell activity, resulting in embryonic demise and reduced serum hCG levels.
2. Interventional Therapy
Radiofrequency ablation is a minimally invasive technique applicable for treating cervical canal pregnancy. It primarily uses thermal effects to induce protein coagulation and necrosis of the affected tissue, followed by uterine evacuation, which facilitates the removal of necrotic tissue.
3. Surgical Treatment
In cases involving vaginal bleeding, embolization therapy may be performed after uterine evacuation to help shrink the gestational sac, improve blood perfusion, and then a subsequent dilation and curettage (D&C) procedure may be carried out.
Besides the above methods, total hysterectomy may also be considered. During recovery, regular follow-up examinations are recommended, along with attention to personal hygiene and avoidance of resuming sexual intercourse too early.
