Introduction
Elderly individuals with diabetes may experience severe body tremors. This could be due to exposure to a cold environment without timely addition of clothing, resulting in physiological shivering. No special treatment is required; symptoms can be relieved by adding clothes and moving to a warmer environment. Alternatively, the tremors might result from hypoglycemia or advanced diabetes causing neuropathy. In such cases, symptoms can be improved through dietary adjustments and medication.

   In general, severe shaking in elderly individuals with diabetes may be a physiological phenomenon or could be caused by factors such as hypoglycemia or diabetic neuropathy. A detailed analysis is as follows:
  1. Physiological Factors
  The elderly person may be exposed to a low environmental temperature and has not added clothing in time. In such cases, skeletal muscles generate heat, but if this is insufficient, involuntary muscle contractions occur, leading to uncontrollable body tremors. This is a normal physiological response that does not require special treatment. Symptoms can be relieved by adding clothing and moving to a warmer environment.
  2. Pathological Factors
  1) Hypoglycemia
  This may result from the elderly patient not strictly following medical instructions—for example, adjusting medication doses without guidance—or from prolonged consumption of nutritionally inadequate foods, which can trigger hypoglycemia and lead to excessive sympathetic nervous system activation. Treatment should be individualized based on the patient's condition, following medical advice for medication use and dietary adjustments. Consuming candy or other sugary foods may help alleviate symptoms. If necessary, patients may require intravenous administration of glucose or medications such as hydrocortisone, as directed by a physician.
  2) Diabetic Neuropathy
  Poor blood glucose control over time may lead to persistently elevated blood sugar levels, resulting in diabetic nerve damage. This condition may also present with additional symptoms such as limb weakness and tachycardia. Patients should take medications such as mecobalamin tablets, oryzanol tablets, and metformin hydrochloride tablets as prescribed.
  Besides the above causes, tremors may also be related to Parkinson’s disease. It is recommended to seek medical evaluation at a hospital to determine the exact underlying cause and receive targeted treatment. If patients experience any discomfort, they should promptly visit a hospital for medical care.
