1. Enterohepatic circulation

After a drug is metabolized in the liver, its metabolites are excreted into the intestine via bile. Some of these metabolites are eliminated, while others are reabsorbed in the small intestine and return to the liver, eventually re-entering the systemic circulation. This repeated entry of the drug into the bloodstream results in a double peak.

2. Dual-site absorption

The drug may be absorbed into the bloodstream at two different locations within the gastrointestinal tract, with differing absorption times, thereby producing the double-peak phenomenon.

3. Differences in components

This phenomenon may arise from the presence of different components in the drug formulation, such as immediate-release and sustained-release components. The immediate-release component enters the bloodstream rapidly, whereas the sustained-release component is absorbed more slowly, leading to the observed double peak.
