Introduction
Cervical conization refers to a cone biopsy of the cervix. This procedure is commonly performed for cervical carcinoma in situ or severe cervical dysplasia. During cervical conization, under hysteroscopic guidance, a resecting loop is used to remove cervical canal tissue, which is then sent to the pathology department for examination.   In general, cervical conization refers to a cone biopsy of the cervix. This procedure is commonly performed for cervical carcinoma in situ or severe cervical atypical hyperplasia. The specific steps are as follows:

  1. Administer intravenous general anesthesia to the patient, position the patient in lithotomy position, disinfect the vulva, vagina, and cervix, and drape the surgical field.

  2. Use a speculum to expose the cervix, then disinfect the vagina, cervix, and external cervical os again.

  3. Dilate the cervix using cervical dilators until fully dilated. Then advance the hysteroscope gradually through the cervical canal. When reaching the uterine fundus, observe the detailed condition of intrauterine lesions via the hysteroscopic system.

  4. Fully extend the resecting loop. When cutting the anterior lip, use the speculum as a fulcrum; when cutting the posterior lip, there is often no definite fulcrum.

  5. Cutting typically begins at the 6 o'clock position and proceeds clockwise.

  6. Tissue specimens taken from high-risk areas under colposcopic guidance should be sent separately for pathological examination.

  7. Remove the hysteroscope and suture the residual tissue to achieve hemostasis, completing the surgery.

  8. Clean the patient’s genital area, abdomen, and upper thighs, and help the patient dress. The physician will provide appropriate advice and preventive measures based on the individual patient's condition.
