Introduction
Misoprostol tablets typically induce bleeding within 6 hours, often accompanied by the passage of large blood clots and the expulsion of the gestational sac. After the gestational sac is expelled, bleeding may persist for 7–10 days. Particularly during the first 3 days, heavy bleeding—similar in volume to a menstrual period—may occur. Only after complete endometrial shedding can residual tissue surrounding the gestational sac be fully expelled.
Misoprostol is a labor-inducing agent, commonly used also as a gastroprotective drug. It is primarily indicated for medical termination of pregnancy in women up to 112 days (16 weeks) after the last menstrual period; however, it must be administered in combination with mifepristone. Additionally, misoprostol is used in the treatment of gastric and duodenal ulcers. Typically, vaginal bleeding begins within six hours after administration. This bleeding is often accompanied by large blood clots and results in the expulsion of the gestational sac.
After expulsion of the gestational sac, bleeding may persist for 7–10 days. Particularly during the first three days, heavy bleeding—similar in volume to a normal menstrual flow—may occur. Only after complete endometrial shedding can residual tissue surrounding the gestational sac be fully expelled, thereby avoiding the need for surgical uterine evacuation (dilation and curettage, D&C).
However, if bleeding persists beyond 10 days, retained products of conception are likely. In such cases, ultrasound examination is recommended to assess whether retained blood or fluid remains within the uterine cavity. Based on the amount and nature of any retained material, treatment may involve either oral medication or surgical D&C. If the gestational sac has not been expelled within 72 hours, pharmacological abortion failure should generally be considered.
This medication must be used strictly under medical supervision. Maintaining a positive mood and minimizing psychological stress are also important. Oral misoprostol promotes uterine contractions, facilitating the expulsion of intrauterine residual tissue. Regardless of current bleeding volume, patients should adhere closely to their physician’s instructions regarding dosage and schedule, and attend all scheduled follow-up examinations.
