How are gallstones treated?
Cholelithiasis (gallstones) can be managed through various approaches, including dietary modification, physical conservative interventions, choleretic drug therapy, minimally invasive stone removal, and surgical cholecystectomy. The choice of treatment depends on stone size and the degree of gallbladder inflammation; individualized management is essential. If persistent severe abdominal pain, high fever, or jaundice occurs, prompt medical evaluation and treatment are required.
1. Dietary Modification: Strictly limit dietary fat intake and choose light, low-fat foods. Maintain regular mealtimes—skipping breakfast must be avoided. Reduce consumption of cholesterol-rich foods, maintain a healthy weight, and avoid overeating or binge eating. This helps stabilize bile circulation, prevent further stone enlargement, and reduce the frequency of biliary colic episodes.
2. Physical Conservative Interventions: During non-acute phases (i.e., absence of active inflammation), moderate physical activity combined with gentle abdominal physiotherapy may promote bile flow through the biliary tract. This approach is primarily suitable for individuals with small stones but cannot directly dissolve large stones. During acute episodes, physical activity should be minimized to prevent stone migration and subsequent biliary obstruction.

3. Choleretic Drug Therapy: For patients with relatively small stones and preserved gallbladder function, choleretic medications may be prescribed. These drugs help regulate bile composition and enhance bile secretion, facilitating the spontaneous passage of tiny stones. However, pharmacological dissolution has limitations—larger stones rarely respond to medication alone. Regular follow-up imaging is essential during treatment to monitor stone changes.
4. Minimally Invasive Stone Removal: When stone burden is low and the gallbladder retains functional value, minimally invasive stone extraction may be considered. Using endoscopic instruments, stones are removed while preserving native gallbladder tissue. Post-procedure, strict dietary control remains necessary, as bile metabolism remains unchanged and recurrent stone formation remains possible.
5. Surgical Cholecystectomy: Indicated for large stones, recurrent biliary colic, or significant gallbladder wall thickening and inflammation. Surgical removal of the diseased gallbladder eliminates the environment conducive to stone formation. After surgery, the body must adapt to altered bile metabolism; accordingly, dietary and lifestyle adjustments are crucial to minimize postoperative discomfort.
Maintain a pattern of frequent, small meals; completely abstain from alcoholic beverages; avoid staying up late or excessive fatigue; and undergo regular follow-up evaluations of the biliary system. Adjust dietary and therapeutic strategies promptly based on evolving clinical status.
