What Causes Genital Itching in Women?
1. Sexually Transmitted Viral Infections
Examples include genital warts (caused by human papillomavirus) and genital herpes. Contagious molluscum contagiosum affecting the vulva typically presents as round papules with central umbilicated depressions and may cause vulvar pruritus. Genital herpes commonly manifests as small, clear, rice-grain-sized vesicles at the urethral meatus and vaginal walls, accompanied by localized itching.
2. Trichomoniasis
Classic symptoms include vulvar and vaginal itching, along with frothy, malodorous vaginal discharge. Scratching may lead to eczematous or dermatitic changes in the vulvar skin. Likewise, candidal vulvovaginitis can also cause genital itching in women, frequently presenting with pruritus, burning sensation, erythema, and edema of the vulvar skin. In severe cases, patients may develop vesicles, papules, pustules, eczematoid erosions, ulcers, or even vulvar pain.
3. Phthiriasis Pubis (Pubic Lice Infestation)
Pubic lice typically adhere to the surface of the vulvar skin or attach to the base of pubic hairs. Their bites, along with their toxic secretions and excretions, induce intermittent vulvar pruritus—ranging from mild to severe—and may result in scaling, secondary eczematous changes, and folliculitis.
4. Lichen Sclerosus
This condition is characterized by vulvar pruritus, sometimes accompanied by burning or pain. The vulvar skin becomes rough and thickened, resembling lichen, with visible excoriations and occasionally mild atrophy. Localized hypopigmentation occurs, causing the labia majora and minora to appear uniformly white. Additionally, during nighttime relaxation of the anal sphincter, pinworms may migrate from the rectum to the external genitalia to mate and lay eggs, thereby irritating the vulvar skin and mucosa and inducing localized pruritus.
