Wrinkles are a woman’s archenemy. As women age, collagen in the facial skin gradually diminishes, causing the skin to become increasingly lax—leading to the untimely appearance of wrinkles. This often causes significant distress for many women. In addition to maintaining diligent daily skincare, individuals with pronounced wrinkles may consider botulinum toxin injections (commonly known as “wrinkle-relaxing injections”). It is not advisable to administer supplementary injections immediately upon the reappearance of wrinkles after an initial treatment. Botulinum toxin is a biological agent; repeated, short-interval injections increase the risk of developing resistance (i.e., diminished therapeutic response). Therefore, it is best to extend the interval between injections to preserve efficacy and prevent resistance. Once resistance develops, it may take 2–3 years before normal drug responsiveness is restored—and during this period, subsequent injections will yield suboptimal results.

1. Facial Expression Stiffness

Many individuals experience excellent results after their first wrinkle-relaxing injection. However, the effect is not permanent: the toxin is typically metabolized by the body within approximately six months to one year, after which wrinkles reappear—requiring repeat injections to maintain results. With repeated treatments, however, patients may develop facial expression stiffness and lose the ability to make broad or expressive facial movements.

2. Allergic Reactions

Some individuals have inherently sensitive skin—termed “sensitive skin.” Such individuals are more prone to allergic reactions following wrinkle-relaxing injections. Additionally, use of non-standard or unapproved injectable agents may also trigger facial allergic responses. In such cases, injection should be discontinued immediately, and appropriate antiallergic medications should be administered promptly to manage symptoms.

3. Strabismus (Eye Misalignment)

Strabismus is a relatively common adverse effect associated with wrinkle-relaxing injections. The primary active ingredient—botulinum toxin—can cause localized muscle paralysis following injection. Frequent injections may lead to abnormal muscular function over time, potentially resulting in strabismus. This side effect is closely related to injection site; patients receiving repeated injections around the periorbital area (e.g., for crow’s feet) are at higher risk.
