For instance, individuals with diabetes often experience reduced immunity, leading to increased smegma production and even secondary fungal infections.

Excessively long or tight foreskin can conceal smegma, preventing natural cleansing. Sebum secreted by the prepuce mixes with urinary residues, forming foul-smelling, whitish “smegma.” While routine cleaning usually suffices for most men with normal anatomy, experts note that numerous factors may contribute to abnormal accumulation—including infectious causes. Unprotected sexual intercourse, for example, may transmit pathogens such as Staphylococcus, Streptococcus, Escherichia coli, Candida albicans, Trichomonas vaginalis, Chlamydia trachomatis, Mycoplasma, other fungi, Neisseria gonorrhoeae, or additional bacterial species.

In daily life, men should regularly wash the foreskin and glans with warm water, maintain regular physical activity to bolster immune function, and thereby help prevent fungal infections that promote smegma accumulation.
