Introduction
At 20 weeks’ gestation, a normal fetal heart rate ranges from 110 to 160 beats per minute. An increased fetal heart rate may be associated with maternal fever or fetal hypoxia. A decreased fetal heart rate may result from certain drug components crossing the placenta and affecting the fetus. Fetal heart cessation is typically a sign of miscarriage or intrauterine fetal demise; its occurrence may be related to drug exposure, adverse environmental factors, or other causes.

At 20 weeks’ gestation, a normal fetal heart rate ranges from 110 to 160 beats per minute (bpm). Fetal tachycardia (increased heart rate), bradycardia (decreased heart rate), or cessation of the fetal heartbeat are all abnormal findings requiring prompt evaluation and intervention. Specific scenarios are outlined below:

     1. Fetal Tachycardia

Fetal tachycardia may be associated with maternal fever or fetal hypoxia. Generally, the more severe the intrauterine hypoxia, the more pronounced the fetal heart rate abnormality. Maternal conditions such as fever or hyperthyroidism can also elevate the fetal heart rate, often exceeding 160 bpm.

       2. Fetal Bradycardia

Fetal bradycardia may result from certain medications taken during pregnancy. When drug components cross the placenta and affect the fetus, they may cause a reduction in heart rate. Persistent bradycardia warrants evaluation for possible congenital heart disease.

      3. Absence of Fetal Heartbeat

The absence of a detectable fetal heartbeat is typically indicative of miscarriage or embryonic/fetal demise. Contributing factors include medication exposure, adverse environmental influences, embryonic chromosomal abnormalities, and endocrine disorders. Preconception health assessments are crucial, enabling early identification of potential issues and timely, appropriate interventions—thereby helping to prevent fetal demise.
