What Causes Pain in the Upper Right Abdomen?
Recently, I’ve been experiencing pain in the right upper abdomen, lasting about an hour before subsiding. The pain is dull and mild—not severe.
What Causes Right Upper Quadrant (RUQ) Abdominal Pain?
Pain in the right upper abdomen may stem from conditions such as cholecystitis, liver disease, or biliary tract disorders. Cholecystitis—particularly common and often associated with gallstones—is a frequent cause. Changes in diet or body position may cause a gallstone to become lodged at the cystic duct opening, obstructing bile drainage from the gallbladder. This obstruction can trigger acute cholecystitis and biliary colic. Since the gallbladder lies beneath the right rib margin, such obstruction typically causes sharp, localized pain in the right upper abdomen. Patients may also experience referred pain to the right shoulder, back, or scapular region. Liver diseases—including acute hepatitis or fulminant hepatitis—can cause RUQ pain due to extensive hepatocyte and hepatic tissue necrosis. Hepatocellular carcinoma (HCC), especially when complicated by tumor rupture or hemorrhage, may also lead to sudden, severe RUQ pain. However, RUQ pain directly attributable to primary liver disease is relatively uncommon. Biliary tract disorders—such as cholangitis or choledocholithiasis—can likewise produce RUQ discomfort. Additionally, duodenal bulb ulcers may manifest as epigastric or RUQ pain.

Therefore, when evaluating a patient presenting with RUQ pain, clinicians should carefully inquire about the nature of the pain (e.g., dull, sharp, colicky), its onset, duration, aggravating or relieving factors, and whether it is constant or intermittent. Correlating this history with physical examination findings—as well as relevant laboratory tests and specialized imaging studies—helps narrow down the likely affected organ or system.
