Difficulty breathing in acute pleurisy may result from a large amount of fluid accumulating in the pleural cavity, creating pressure. This pressure can compress the lungs, leading to partial or complete lung collapse (atelectasis). Atelectasis causes shortness of breath and may trigger coughing. Additionally, acute pleurisy can be caused by infections of the lungs due to various pathogens such as bacteria, viruses, or mycoplasma, resulting in symptoms like chest pain and difficulty breathing. In some cases, mediastinal abscess, subphrenic abscess, or liver abscess may directly rupture into the pleural cavity, causing acute pleurisy and subsequent breathing difficulties.
