There are many causes of heavy menstrual bleeding at age 48, with the most common being physiological changes. Due to perimenopause, hormonal imbalances occur—estrogen levels decrease while progesterone remains relatively high. This can lead to increased menstrual flow, prolonged periods, or even reduced bleeding and delayed menstruation, all of which are considered normal. Some women may have endometrial polyps, uterine fibroids, or cervical polyps, which can cause excessive menstrual bleeding or prolonged spotting; an active ultrasound examination is needed to determine the exact cause. If there is severe bleeding due to uterine fibroids, surgical treatment may be required.
