Severe cases: If symptoms of orchitis are severe—such as significant testicular swelling, intense pain, accompanied by systemic symptoms like high fever and chills—or if the inflammation has involved the reproductive tract leading to serious complications such as hematospermia, surgical intervention may need to be considered. The usual surgical approach is incision and drainage to remove the infectious focus and pus, helping to resolve the inflammation.
