Whether prostatitis can resolve spontaneously mainly depends on its type and severity.

1. Acute bacterial prostatitis: Caused by bacterial infection, this type usually requires antibiotic treatment. Without treatment, it is unlikely to resolve on its own and may lead to worsening symptoms or progression to chronic prostatitis.

2. Chronic bacterial prostatitis: Also caused by bacterial infection, but with a longer disease course and greater treatment challenges. Long-term antibiotic therapy and adjunctive treatments are generally required, making spontaneous resolution unlikely.

3. Severe prostatitis: If symptoms are severe, such as high fever, intense pain, or urinary tract symptoms, or if complications such as seminal vesiculitis or urethritis are present, spontaneous resolution is unlikely. Prompt medical intervention is required, including medication and physical therapy, to alleviate symptoms and control the condition.

4. Asymptomatic inflammatory prostatitis: Patients typically exhibit no obvious symptoms and the condition is often discovered during routine physical exams or investigations for other diseases. If the inflammation is mild, it may resolve spontaneously, although it can also persist without resolution.
