The treatment options for thrombocytopenic purpura mainly include medication therapy, surgical treatment, and other therapeutic approaches. Regarding medication therapy, glucocorticoids such as prednisone and dexamethasone are the first-line treatments, as they can reduce capillary permeability and inhibit the production of platelet antibodies. For patients with severe bleeding, pulse therapy with high-dose dexamethasone or methylprednisolone can be administered. Additionally, intravenous immunoglobulin (IVIG), anti-D immunoglobulin, rituximab, thrombopoietin (TPO) or TPO receptor agonists (such as eltrombopag and recombinant human thrombopoietin), and immunosuppressive agents (e.g., cyclosporine, vincristine) may also be used in treatment. Concerning surgical intervention, laparoscopic splenectomy is suitable for patients with chronic thrombocytopenic purpura who have not responded satisfactorily to medical therapy. Other treatments, such as plasma exchange, can remove adsorbed antibodies from platelets and thus reduce progressive platelet destruction.
