Uneven endometrial echogenicity refers to the finding during an ultrasound examination that the image of the endometrium is not uniform, possibly containing areas with varying intensities of echo. This condition is usually associated with several factors:

1. Endometrial hyperplasia: During the normal menstrual cycle, the endometrium thickens in response to hormonal changes, preparing for implantation of a fertilized egg. If this thickening process is uneven, it may result in uneven echogenicity.

2. Hormonal influences: Endocrine disorders, such as an imbalance between estrogen and progesterone, may lead to excessive or insufficient localized endometrial proliferation, causing differences in echogenicity.

3. Endometrial polyps: Small endometrial polyps may appear on ultrasound as regions of uneven echogenicity.

4. Inflammation or infection: Endometritis or other reproductive system infections can cause localized inflammatory responses, leading to uneven echogenicity.

5. Tumors: Although less common, benign or malignant tumors of the endometrium may also cause such findings.

6. Other causes: Incomplete endometrial shedding, mucous cysts, and similar conditions may also affect the echogenic characteristics.
