I had some discomfort in my kidney area recently and was diagnosed with an 0.8-sized stone. My abdomen suddenly started hurting severely these past few days, and the doctor at the hospital told me that both of my kidneys are experiencing hydronephrosis (kidney swelling due to urine buildup). I would like to ask, is having a 0.8 kidney stone with bilateral hydronephrosis considered serious?

In general, a 0.8 cm kidney stone is considered to be of moderate size and may affect the normal outflow of urine, leading to hydronephrosis.

The severity of bilateral hydronephrosis depends on the degree and duration of the accumulation, as well as whether there are accompanying symptoms such as infection, hypertension, or renal dysfunction. Short-term mild hydronephrosis may not significantly affect kidney function, but long-term or severe hydronephrosis may lead to renal parenchymal atrophy and impair kidney function.

When a kidney stone reaches 0.8 cm in size, it is considered relatively large, and the likelihood of spontaneous passage is low. Treatment usually involves extracorporeal shock wave lithotripsy (ESWL) to fragment the stone so it can be passed out of the body with urine. This can effectively relieve symptoms of hydronephrosis. After lithotripsy, diuretics or other medications may be prescribed based on individual conditions to increase urine output, promote stone passage, and alleviate symptoms of hydronephrosis.
