Recently, I noticed that I have developed an anal fissure. However, I haven't experienced any bleeding—only persistent pain. Anal fissure is a common anorectal disease, primarily characterized by a full-thickness tear in the anal mucosa that forms a chronic ulcer. The most typical symptoms of anal fissures are pain and rectal bleeding. Some patients may experience pain without bleeding, which can occur when the fissure is small and does not damage blood vessels, or when the tear is deep and bleeding is not easily observed. Pain occurs because defecation increases pressure in the anal canal, causing the fissure to tear further and intensifying the pain.

For anal fissures without bleeding but with persistent pain, patients are advised to increase dietary fiber intake, drink sufficient water daily, and perform warm sitz baths several times a day to help relieve pain. Additionally, under a doctor's guidance, topical ointments with analgesic and antibacterial properties may be used. If the pain persists or is accompanied by other symptoms, prompt medical attention is recommended.
