In general, whether to use cold or heat compress for hand swelling after intravenous infusion depends on the specific situation. Cold compress is usually recommended within the first 24 hours. If more than 48 hours have passed, heat compress is generally advised. During the period of hand swelling, patients should avoid consuming spicy and irritating foods such as chili peppers and raw garlic to reduce irritation to the swollen area.

Generally, whether to use cold or heat application for hand swelling after intravenous (IV) infusion depends on the specific situation. Cold compress is usually recommended within the first 24 hours, while heat compress is generally advised after 48 hours.

Hand swelling after IV infusion typically occurs when the infusion rate is too fast or the medication used is highly irritating, increasing the permeability of the arm's veins and leading to phlebitis. If swelling is noticed during the infusion process, the IV should be stopped immediately at the swollen site, and cold compress applied. Cold compress helps constrict the blood vessels, reduces further fluid leakage into the surrounding tissue, and alleviates swelling at the infusion site.

About 24 hours after the infusion, capillary fluid leakage usually resolves. After 48 hours, applying a warm compress can be beneficial. Placing a warm towel over the swollen area improves blood circulation under the skin, promotes faster absorption of edema, and thereby relieves hand swelling symptoms.

If neither cold nor heat application effectively reduces the swelling, medications such as melilotus officinalis extract tablets or Tongsongling may be used under a doctor’s guidance to help reduce inflammation and swelling.

Patients are also advised to avoid consuming highly spicy or irritating foods—such as chili peppers and raw garlic—during the period of hand swelling to minimize irritation to the affected area.
