Cerumen impaction is a common external auditory canal condition caused by accumulation of secretions from the ceruminous and sebaceous glands lining the ear canal. Under normal circumstances, cerumen serves a protective function—helping to prevent infection and maintain ear canal health. However, when excessive cerumen causes discomfort or obstruction, removal becomes necessary. If impacted cerumen is too hard to extract with forceps—or if the patient is uncooperative—cerumenolytic drops should first be administered to soften the wax. After three days, the ear canal should be re-evaluated and irrigated as needed. The primary active ingredient in most cerumenolytic solutions is sodium bicarbonate, which effectively softens and dissolves cerumen, facilitating subsequent irrigation.
