Cold hands and feet accompanied by sweating may be related to excessive emotional tension—for instance, being startled can cause sudden constriction of the capillaries in the extremities, leading to cold hands and feet, while simultaneously stimulating sweating in these areas. If coldness in the hands and feet persists and is unrelated to being startled, it is commonly associated with kidney yang deficiency, qi and blood deficiency, or hypothyroidism.
