However, PHN is notoriously difficult to treat, and drug therapy must always be conducted under strict physician supervision.

Herpes zoster (shingles) is caused by reactivation of the varicella-zoster virus, which remains latent in the dorsal root ganglia neurons of the spinal cord. When immune defenses weaken—due to factors such as excessive fatigue, infection, emotional stress (e.g., anger), colds, or fever—the virus reactivates, replicates, and travels along nerve fibers to the skin. This triggers inflammatory responses in both the affected nerves and overlying skin, resulting in neuropathic pain. Thus, pronounced neuralgia is the fundamental manifestation of postherpetic neuralgia.
