Introduction
There is no single best exercise method for avascular necrosis of the femoral head. However, exercises such as leg lifts while lying down, internal and external rotation movements, and hip abduction and adduction in a seated position can be beneficial. Additionally, activities like swimming and cycling may also help promote recovery. It is important to choose appropriate exercise intensity and duration, progress gradually, and avoid overexertion, which could worsen the condition.

There is no single best exercise method for femoral head necrosis. However, exercises such as the supine leg lift, internal and external rotation, and seated abduction-adduction can be beneficial.

1. Supine Leg Lift

The patient lies flat or face down on the bed and alternately performs straight leg raises with each leg. This helps strengthen the hip joint and leg muscles and prevents muscle atrophy. It is recommended to perform 200 repetitions daily, divided into 3–4 sets.

2. Internal and External Rotation

The patient begins in a supine position with both lower limbs extended, feet shoulder-width apart, and hands placed at the sides of the body. Using the heels as an axis, rotate the toes and lower limbs inward and outward for 5 to 10 minutes, focusing more on the side with greater functional limitation.

3. Seated Abduction-Adduction

The patient sits on a backless stool, holding the knees with both hands and placing the feet shoulder-width apart. Slowly bend forward until the torso reaches just above the knees, then lean backward to the maximum extent possible. Perform this movement slowly and repeatedly, completing 100 repetitions per set, with two to three sets per day.

In addition, activities such as swimming and cycling may also be suitable and can aid in recovery. It is important to choose appropriate training intensity and duration, progressing gradually to avoid overexertion and worsening of the condition.
