Introduction
In general, the surgical indications for uterine fibroids should be comprehensively evaluated based on the patient's age, fertility requirements, clinical symptoms, and other factors; there is no fixed standard. Surgery is usually required if uterine fibroids cause excessive menstrual bleeding, abnormal uterine bleeding, or anemia that does not respond to medical treatment. Patients are advised to choose a treatment option according to their doctor's recommendations.

Generally, the surgical indications for uterine fibroids should be comprehensively evaluated based on the patient's age, fertility requirements, clinical symptoms, and other factors. There is no fixed standard. Doctors usually recommend surgery under the following 10 circumstances:

1. Excessive menstrual bleeding, abnormal uterine bleeding, or anemia caused by uterine fibroids that do not respond to drug treatment.

2. Infertility or recurrent miscarriages caused by fibroids.

3. Fibroid diameter greater than or equal to 4 cm, but with fertility desires.

4. Large fibroids causing compression of the bladder or rectum, leading to a series of symptoms that significantly affect daily life.

5. Severe abdominal pain, dyspareunia (pain during intercourse), chronic pelvic pain, or acute abdominal pain due to torsion of a pedunculated fibroid.

6. Continued growth of uterine fibroids after menopause, raising suspicion of malignant transformation.

7. Rapid growth of fibroids during pregnancy, raising suspicion of malignant transformation.

8. Red degeneration and necrosis of fibroids during pregnancy unresponsive to conservative treatment.

9. Torsion or secondary infection of a pedunculated fibroid during pregnancy unresponsive to conservative treatment.

10. Fibroid enlargement during pregnancy causing significant compression of adjacent organs and severe symptoms.

It should be noted that treatment of uterine fibroids must be individually tailored according to the patient’s age, fertility plans, symptoms, and the location and size of the fibroids. Blindly choosing medication or surgery is not recommended. Patients are advised to follow their physician's recommendations when deciding on a treatment approach.
