Introduction
Blood clots after needle aspiration of an ovarian chocolate cyst may be a normal phenomenon, but could also result from improper procedure or adenomyosis. Symptomatic management should be based on the patient's specific condition. The following is a detailed analysis: Ovarian chocolate cysts are a form of endometriosis. If intense physical activity occurs during menstruation, it may lead to retrograde menstrual flow into the abdominal cavity.

Ovarian chocolate cyst aspiration followed by the presence of blood clots may be a normal phenomenon, but could also result from improper procedure or adenomyosis. Symptomatic management should be based on the patient's specific condition. The detailed analysis is as follows:

1. Normal phenomenon

Ovarian chocolate cysts are lesions associated with endometriosis. During menstruation, intense physical activity may cause retrograde menstrual flow into the abdominal cavity. Displaced endometrial tissue can then implant and grow within the abdomen. This ectopic endometrial tissue responds to ovarian hormonal cycles and sheds during menstruation, but the resulting blood cannot be discharged from the body, leading to the formation of ovarian chocolate cysts. After the procedure, retained blood within the cyst is expelled. If the volume of blood is large and coagulation occurs, the appearance of blood clots following cyst aspiration is considered a normal occurrence.

2. Improper procedure

If surgical technique during the procedure is inadequate—resulting in a large wound surface or damage to local blood vessels—excessive bleeding may occur, leading to clot formation. If this symptom persists, patients should seek medical attention for appropriate hemostasis.

3. Adenomyosis

If a patient’s endometrium is abnormally stimulated and damaged, basal layer cells may invade the myometrium, accompanied by hypertrophy and hyperplasia of surrounding cells, resulting in adenomyosis. During menstruation, blood sinuses open and uterine contractions become frequent. If blood sinuses are compressed, menstrual flow may increase, causing this symptom. It is recommended that patients seek hospital care, where treatments such as excision of adenomyotic lesions or myometrial resection may be performed.

Besides the above-mentioned causes, vaginal mucosal injury, endometrial polyps, or uterine fibroids may also contribute. It is advised to maintain regular作息 (daily routines), practice good genital hygiene, and moderate the frequency of sexual intercourse.
