1. Physical Cooling

When fever results from food retention, immediate administration of antipyretics may impair the body’s natural defense against pathogens. If the temperature remains below 38.5°C, physical cooling methods are recommended. Gently wipe the infant’s forehead, palms, soles, back, and chest with lukewarm water to promote heat dissipation; a lukewarm bath is also beneficial. Dress the infant in loose, lightweight clothing to facilitate heat loss. Encourage increased fluid intake to replace fluids lost through fever and to support metabolism and internal heat clearance.

2. Consumption of Liquid Foods

3. Abdominal Massage

Constipation-induced abdominal distension and pain can be alleviated through gentle abdominal massage. Using the palm of the hand, perform clockwise circular massage around the navel. When encountering a firm area (possibly impacted stool), apply slightly increased downward pressure to stimulate defecation. Additionally, lightly massage the perianal area with fingertips lubricated with baby oil to encourage bowel movement.

4. Rational Use of Medications

The primary therapeutic goal for fever due to food retention is resolving food stagnation. Mild traditional Chinese patent medicines suitable for infants may be administered to promote bowel movement; once constipation resolves, the fever often subsides. While addressing food retention, closely monitor the infant’s symptoms and temperature. Antipyretics are generally unnecessary if the temperature remains below 38.5°C and no other concerning symptoms are present—increased hydration and physical cooling suffice. For temperatures ≥38.5°C, use pediatric-specific antipyretics. Stronger antipyretics may be considered for temperatures ≥39°C, but dosage must strictly adhere to recommended guidelines to avoid overdose.

5. Appropriate Physical Activity

Encourage age-appropriate physical activity to enhance gastrointestinal motility and aid digestion, thereby helping resolve food retention. Exercise also stimulates sweat gland secretion, facilitating heat dissipation through perspiration and contributing to fever reduction.
