Hepatocellular carcinoma (HCC) is a common chronic disease. Typically, liver cancer is classified into two main types: primary liver cancer and secondary (metastatic) liver cancer. Some patients diagnosed with liver cancer may be uncertain how to distinguish between these two types. So, what are the key differences between primary and secondary liver cancer?
Differences Between Primary and Secondary Liver Cancer
Primary liver cancer originates from hepatocytes or intrahepatic bile duct epithelial cells within the liver itself. In contrast, secondary liver cancer arises when malignant tumors from other organs metastasize to the liver. This fundamental distinction reflects differences in tissue origin. Moreover, their biological behaviors differ significantly: primary liver cancer exhibits biological characteristics typical of hepatic malignancies, whereas metastatic liver cancer retains the histological and biological features of its original (primary) tumor.

Early detection is critical for both primary and secondary liver cancers. Once a tumor develops—particularly a metastatic one—the presence of a primary tumor elsewhere in the body followed by hepatic metastasis, and subsequently spread to other organs, indicates advanced disease staging. If metastasis to the liver is detected early—ideally at the earliest stage, when only precancerous changes or microscopic lesions are present—surgical resection before full-blown tumor formation can significantly improve outcomes. Waiting until a clinically apparent tumor or secondary hepatic lesion develops is far less effective.

Consequently, treatment strategies also differ. For secondary liver cancer, management involves not only standard liver-directed therapies—including surgical resection—but also definitive treatment of the primary tumor site. Postoperative sensitivity to chemotherapy agents, as well as the selection and design of chemotherapy regimens, depend directly on the characteristics of the primary tumor.
