Acute orchitis can be managed either conservatively or surgically. Conservative management primarily involves pharmacotherapy, typically with antimicrobial agents. Antibiotic selection should be guided by bacterial culture and antibiotic susceptibility test results to ensure use of an appropriate, sensitive agent. Commonly used antibiotics include cephalosporins and fluoroquinolones; administration must strictly follow the physician’s instructions. If pharmacotherapy proves ineffective, surgical intervention may be considered—such as drainage, decompression, or excision of affected tissue—while preserving normal testicular function.
